WHAT IS AN ELECTRONIC HEALTH RECORD? An electronic medical record (EMR) or an electronic health records (EHR) is an individual medical record in digital format. It is accessed on a computer, often over a network. Both electronic health record (EHR) and electronic medical record (EMR) have gained widespread use, with some health informatics users assigning the term EHR to a global concept and EMR to a discrete localised record.In practice the terms EHR and EMR are used interchangeably.  A “Complete EMR system" has all four basic functions deemed minimally necessary for a full EMR: computerized orders for prescriptions( to eliminate illegible handwriting and catch errors), computerized orders for tests, reporting of test results, and physician notes. Used by 9% of doctors in the US.  The "born digital" record has been electronic from the start, entered into a database, or transcribed from an electronic tablet or notebook PC.  A "digital format record" was originally produced in a paper or other hardcopy form (X-ray film, photographs, etc.) that have been scanned or imaged and converted to a digital form.  Health Information Technology is an even broader term that describes any computer-based electronic aid to healthcare delivery. such as the practice management system which supports the electronic medical record.  A personal health record (PHR) is medical information that is in the possession of an individual patient (or patient's non-professional caregiver).(nb if it is on the internet it is not in the possession of a patient alone).  Continuity of Care Records (CCR) are brief summaries of recent healthcare encounters, including only significant data. It is meant to enable a rapid assessment of the patient's overall health and recent visits to healthcare providers. It is rarely used currently and not standardised.

What data does an electronic medical record include? Patient demographics. Medical history, examination and progress reports of health and illnesses. Medicine and allergy lists, and immunization status. Laboratory test results. Radiology images (X-rays, CTs, MRIs, etc.) Photographs, from endoscopy or laparoscopy or clinical photographs. Medication information, including side-effects and interactions. Evidence-based recommendations for specific medical conditions A record of appointments and other reminders. Billing records. Advanced directives, living wills, and health powers of attorney However, CURRENTLY INCLUDED IN YOUR MEDICAL RECORDS IS ALMOST ALL YOUR PERSONAL INFORMATION,FROM YOUR SOCIAL SECURITY NUMBER, TO COPIES OF YOUR DRIVERS' LICENCE, CREDIT CARD NUMBERS , EMPLOYER, FAMILY MEMBER DETAILS AND PERSONAL INFORMATION.

Doctors in general are technologically savvy people. Many have tried to implement EMRs. An estimated 30% of EHR implementation attempts have failed over the past few years. EMRs are significantly expensive , time consuming to evaluate and implement, do not improve physician workload, but rather add to it , and THE SYSTEMS FAIL. Just about every physician in private practice has had the system fail, from power outages, to disasters, to human error. The systems save the expense of a clerk but make the doctor do the work of a typist and coder. Primary care is being asked to implement systems at the cost of a year's salary when physicians are making less and less.
